About half of the world's population lacks access to basic sanitation facilities ðWatkins 2006; UNICEF/WHO 2012Þ. Lacking toilets, billions of people across the world respond to nature's call in fields, ditches, rivers, and road-sides—a practice called " open defecation, " which potentially is a major contributor to the global burden of diseases ðPrüss-Üstün and Corvalán 2006; Prüss-Üstün et al. 2008Þ. Although there has been growing attention to sanitation within public health and development communities ð2008 was the International Year of SanitationÞ, there is little careful empirical research on this subject.
